Program description. By opting in, you agree to receive text messages from Rosemary, a service of Affirm Health Inc., related to your advance care planning program, including scheduling, appointment reminders, and care coordination.

Message frequency. Message frequency varies based on your care needs and program participation.

Fees. Message and data rates may apply according to your mobile carrier plan. Rosemary does not charge for text messages.

Opt-out. You can cancel at any time by texting STOP. After you send STOP, we will confirm your unsubscribe status by SMS and you will no longer receive messages. To rejoin, reply START.

Help. For assistance, reply HELP to any message, email support@affirmhealth.com.

Carriers. Carriers are not liable for delayed or undelivered messages.
